No this isn't an application form for a dating agency...and no, we're not just being nosy.
In fact Hull City Council is looking to recruit 5,000 people from all different ages, areas, creeds and colours to form a huge 'citizens panel' which will help improve council services.
It's hoped the panel will be as representative as possible of the demographic make-up of the city as a whole.
Its members will be contacted around three or four times a year and asked their views on council services and initiatives – so that improvements can be made in all the right areas.
So if you want the chance to influence future decision making at the council and to play a part in making Hull a better place, then fill out the form below or do it on-line.
"The best way to achieve great customer service, is by understanding what customers think of services and how they want to be treated," says Cllr Abigail Walker, Portfolio Holder for Customer Services.
"Great customer service is about understanding what people want, being approachable and really listening.
People who use council services should get excellent service every time, and although that might not always happen, it's vital to involve customers in order to improve."
Bransholme does the business
A smaller yet almost identical project has already brought some amazing results on Bransholme – and resulted in 25 tonnes of rubbish being collected as well as 50 illegal nuisance motorbikes being seized over a six-month period.
The Bransholme Community Panel, set up last year, involved more than 700 local people who gave their views about local services and stated what they wanted and needed in their community.
"Almost three-quarters of people were concerned about nuisance motorbike riding, and more than half thought litter was a problem – so these became our areas for priority action," says Andrew Parkinson, community participation officer with the council's North Carr Area Team.
"Panel members could choose how they answered questions whether it was by email, post or someone ringing them on the phone.
"At Christmas time they all received a Christmas card containing feedback on what had been done about problems people were worried about."
